A. The incident
1. At about 1154 hours
(local time) on 10 September 2002, the Hong Kong registered bulk
carrier Hui Shun Hai rammed the quay No. 36 whilst berthing alongside
quay No. 35 at Port of Paljassaare, Estonia.
2. The bulbous bow of
Hui Shun Hai sustained an indent of about 15cm and the hull plates
on the starboard side also sustained an indent of about 8 cm. Quay
No. 36 sustained a damage to its concrete structure at an area of
8 metres x 2 metres at the edge of the quay between bollards No.
11 and 12. Three rubber fenders of 400 x 200 mm were crushed. No
casualties were reported.
B. Findings
1. The investigation
has revealed that the cause of the accident was the error of judgment
of the master and the pilot of Hui Shun Hai in manoeuvring the
vessel in the confined basin where little margin for error is
permitted.
2. The contributory
factors to the error of judgment are that the headway of Hui Shun
Hai was probably excessive that full astern engine was unable
to counteract it with immediate effect; tugs were not made fast
to Hui Shun Hai to exert a braking force to the excessive headway;
and the port anchor was probably dropped too late to stop the
headway of Hui Shun Hai.
C. Lessons
1. While manoeuvring
a large vessel in a confined basin where little margin for error
is permitted, the persons involved in manoeuvring should ensure
the headway of the vessel was under control including the use
of anchor at the correct moment and the tugs were effectively
deployed to assist the manoeuvring.
