A Sailor Of A Locally Licensed Motor Tanker Died
After Fallen Overboard While Vessel Underway
A.   The Incident

At about 0815 hours on 18 April 2002, a sailor of the locally licensed motor tanker "HENG TAT" (恆達), was reported to have fallen overboard in the vicinity of Sulphur Channel at 0.15 nautical mile south of Green Island while the vessel was proceeding to an ocean going ship at harbour mooring buoy B31 for discharging of diesel oil.  Four days later the body of the deceased was recovered from the water south of Tsing Yi Island.

B. Findings

1. The main contributory factor to this fatal incident appeared to be the absence of guard rail at the opening section at the starboard side of the vessel, which was taken off by the shipˇs crew to facilitate bunker hose connection.

2. The wet deck surface and the laying of bunker hose on deck were also considered as contributing factors to the incident, which had aggravated the danger of trip and fall situation.

C. The Lessons

The crew working on board local vessels should be aware of the danger of guard rail opening on deck.  When a section of the railing is taken off to facilitate vessel operation, the opening should at least be fenced off by temporary arrangement, e.g. by a removable chain link, to prevent people from falling over the side accidentally, especially during passage.
